96 FF3-06 Annular Conjunctival Autograft: A ring of hope from the fellow eye Sudipta SARKER Bangladesh This video will show annular conjunctival autograft (ACAG) is a safe and effective technique for managing severe unilateral limbal stem cell deficiency following chemical injury. It provides uniform stem cell distribution and sustained epithelial stability compared with conventional simple limbal epithelial transplantation (SLET). The surgical technique involved 360° conjunctival excision 2 mm from the limbus, then epithelial debridement. A 3-4 mm annular conjunctival graft containing limbal stem cells (excluding Tenon’s tissue) was harvested from the fellow eye. The graft was placed circumferentially over the bare sclera of the affected eye and secured using fibrin glue, with additional interrupted sutures when required. An amniotic membrane and bandage contact lens were applied to the cornea. Finally, the donor eye was reconstructed. The use of pre-prepared (after removal the epithelial and endothelial cells and devitalize intrastromal cells through terminal irradiation) allogenic tissue for CAIRS is expensive and may not represent the ideal parameters for the individual patient. Local donor tissue allows flexible CAIRS customization and may offer biological and handling advantages. This video provides a practical guide for surgeons considering CAIRS adoption through customized CAIRS preparation using locally sourced donor corneal tissue using different femtosecond laser platforms. Delta corneal diameter demonstrated a strong correlation with postoperative vault prediction error (r = 0.9). The AI decision-tree model achieved an area under the curve of 0.9, indicating high predictive accuracy. This approach enables improved vault prediction and enhances refractive safety, supporting the role of AI-driven 3D corneal metrics in next-generation ICL sizing.
